What Is Wound Healing?
Wound healing is a testament to the body’s remarkable biological process of repairing tissue damage. It involves a series of regulated phases that restore the skin’s protective barrier and underlying structures. This process supports functional recovery and helps prevent infection, scarring, and tissue loss.
Healing occurs in a series of overlapping but distinct stages, each contributing to tissue repair:- Hemostasis: Blood vessel constriction occurs to limit blood loss, and clot formation initiates to close the wound site.
- Inflammation: Immune cells clear the wound site of damaged tissue, cellular debris, and microbes.
- Proliferation: Tissue regeneration begins as fibroblasts generate collagen, and angiogenesis occurs to deliver oxygen and nutrients to the healing area.
- Remodeling: The newly formed tissue gradually strengthens. Collagen fibers reorganize, and the wound matures over time.
Causes
A wide range of factors can disrupt the wound healing process. These may be local, directly affecting the wound site, or systemic, involving the body’s overall health and immune function.
Common causes of delayed wound healing include:
- Poor circulation: Reduced blood flow limits oxygen and nutrient delivery, slowing tissue regeneration.
- Diabetes: Elevated glucose levels impair immune function and cellular activity in wound repair.
- Chronic inflammation: Excessive or prolonged inflammatory response interferes with tissue remodeling.
- Nutritional deficiencies: Inadequate intake of protein, vitamin C, zinc, and amino acids, particularly arginine and glutamine, can inhibit collagen synthesis and cellular repair.
- Infection: Bacterial contamination activates the immune defense system, disrupting healing and damaging healthy tissue.
- Medications: Some drugs, such as corticosteroids or chemotherapy agents, suppress the immune system or cell proliferation.
- Advanced age: With age, cellular turnover slows, making older adults more susceptible to non-healing wounds that require special attention.
Chronic conditions such as venous insufficiency, lymphedema, and autoimmune disorders can also compromise the healing process.
Symptoms
Signs of a wound that is healing normally include:
- Gradual reduction in size
- Decreased drainage
- Development of granulation tissue
- Minimal redness or swelling
In contrast, impaired wound healing may present with:
- Persistent pain or inflammation
- Increased or foul-smelling discharge
- Necrotic or discolored tissue
- Wound edges that do not close over time
- Signs of infection, such as localized heat, tissue swelling, or fever
Chronic wounds that do not progress through the healing stages within 4 to 6 weeks are considered non-healing and should be evaluated by a medical provider.
Treatments
Medical intervention for wound healing focuses on addressing the underlying cause, removing barriers to tissue repair, and stimulating biological regeneration. Regenerative therapies, with their potential to accelerate healing in both acute and chronic wounds, offer a beacon of hope in wound care.
Platelet-Rich Plasma (PRP)
PRP therapy involves isolating and concentrating platelets from a patient’s blood, then injecting them into the affected area. Platelets release growth factors that influence every stage of healing, especially the proliferative and remodelling phases.
These growth factors promote:
- Fibroblast activation and collagen synthesis
- New blood vessel formation (angiogenesis)
- Migration of reparative cells to the wound site
Clinical studies suggest that PRP may support wound healing in chronic wounds, such as diabetic foot ulcers and pressure ulcers. Its autologous nature lowers the risk of rejection or immune-related complications.
Stem Cell Therapy
Stem cell-based therapy introduces undifferentiated cells that can develop into multiple tissue types, assisting in replacing damaged cells at the wound site. In particular, mesenchymal stem cells (MSCs) have been studied for their potential role in chronic wound repair.
Benefits of MSCs in wound healing include:
- Modulation of inflammation
- Secretion of bioactive molecules that stimulate tissue repair
- Angiogenic signalling to restore blood supply
Research findings show that MSCs support cell proliferation, immune regulation, and scar minimization. When applied topically or via injection, stem cells may help restart the healing cascade in wounds.
Additional Interventions
Wound care may also include mechanical debridement, infection control, and moisture management, depending on the wound’s condition. Compression therapy may be used in venous wounds, and offloading is necessary for pressure injuries. Supportive strategies, such as nutritional optimization, are often integrated into a comprehensive care plan to address amino acid or micronutrient deficiencies that can impair healing.
